Contract Vaccinator Information

In order to contract as a vaccinator for the mass H1N1 vaccination effort, the following

must be completed:
  • Complete and submit the application  
  • Complete the online training 
  • Schedule and complete the skills assessment    
  • Meet with the project manager in order to complete the application process         
Step 1:  Click on this link to download and complete the application.  Once completed,
please fax to (979)864-3694, Attn: C. Reed-Boston or e-mail to chaseyrb@brazoria-county.com.  The application must be completed in order to be processed.  Your license
will be verified upon receipt of the application and must be current and in good standing.
Step 2:  Click on this link to complete the online training.  This is required in order to
schedule the skills assessment.  Once you have completed the online training, submit the
certificate via fax to (979)864-3694, Attn: C. Reed-Boston or e-mail to chaseyrb@brazoria-county.com.
Step 3:  Schedule the skills assessment.  If you are currently employed as an immunization nurse, the skills assessment can be waived.  Please contact Jan Prejean, RN at janp@brazoria-county.com for scheduling or to discuss waiver.
Step 4:  After you have completed the skills assessment or obtained a waiver, you will meet with the project manager to complete the application process and sign a contract.  At this meeting you will be required to complete a W-9 and provide an additional form of photo identification.
Step 5:  The scheduling will then be done online as events are scheduled.
